Health Records

Student health records and emergency contact information will be maintained at the school where the student attends. 

For a student that has any special needs for busing (including behavior management, physical-motor impairments requiring a lift bus or specialized seating, communication disorder or health and physical needs) a transportation plan is needed;  please complete a transportation plan.
